《麻药王》以上世纪70年代的韩国釜山为背景，秘密讲述了宋康昊饰演的李头森从一个毫无底线的走私犯崛起为传奇毒枭的故事。故事原型是上世纪70年代靠走私起家，秘密后跻身毒品制造和贩卖行业并崛起的一代毒枭，导演表示，李头森虽然是虚构人物，但是利用实际存在过的大毒枭作为原型，经过几年的资料调查才完成了剧本。
